Requirements:

1. ACTUAL PASSPORT

You must submit your actual passport. The visa will be stamped on a page in your passport. Your passport must have at least six months of validity left beyond your intended stay in Bolivia, and must have at least one completely blank visa page. 2. VISA APPLICATION FOR BOLIVIA

Click here to download Visa Application Form for Bolivia. Form must be TYPED. Download and save the form on your computer before filling. Fully complete and sign the form.

3. PHOTOGRAPH

Submit one professional 2″ x 2″ color passport photo. Snapshot, photocopy and computer picture are not accepted. Photo must be:

  • Taken within six months to reflect your current appearance;
  • Taken in full-face front view directly facing the camera. Full face must be visible. Head must be centered and not tilted;
  • Do not wear eyeglasses in the photo;
  • Clear. Blurry, underexposed, overexposed and low quality photos are not accepted;
  • With a plain white or off-white background;
  • With a neutral facial expression and both eyes open.

4. FLIGHT ITINERARY

Computer generated flight itinerary from the airline or travel agency, or copy of airline tickets. Non-refundable tickets or reservations should not be purchased until visa and passport are secured and in your possession.

5. LETTER OF INVITATION

If visiting relatives or friends, a signed letter of invitation from the host explaining the relationship to the applicant and the purpose of visit. In addition, the letter of invitation must indicate length of stay, place of stay, and contact information.

6. PROOF OF LODGING

Copy of hotel confirmation. A letter from inviting family member or friend is required if they are providing accommodations to the applicant.

7. PROOF OF SUFFICIENT FUND

Provide evidence of sufficient fund to support your stay. Example: copy of most recent bank statement with account number blacked out.

8. MINORS

  • Minor under age of 18 must provide a notarized letter of consent signed by both parents or guardian authorizing the consulate to issue a visa to the minor. A copy of the minor's birth certificate and guardianship or custody agreement documents are also required.
  • Signature from both parents or guardian are required on the visa application form, along with the signature of the minor if applicable.
  • Proof of sufficient fund will be the most recent bank statement from the parents or guardian.

Processing time commences from the day the application has been submitted to the consulate. Saturdays, Sundays, consulate scheduled holidays, and mailing do not count as a processing day.

  • $99.00 per visa – 9 to 12 business day processing
  • $129.00 per visa – 5 to 8 business day processing

12. RETURN FEDEX EXPRESS

  • $32.00 FedEx 1 to 3 passports
  • $42.00 FedEx 4 to 6 passports or overweight package
  • $52.00 FedEx to Alaska, Hawaii and Puerto Rico, or Saturday Delivery

4. BUSINESS LETTER

Letter from employer on company letterhead introducing the applicant, indicating the applicant's employment status and position held in the company, and clearly stating purpose of visit. The business letter must also indicate who will be financially responsible for the applicant and must provide detailed contact information in Bolivia.

5. CERTIFICATE OF GOOD CONDUCT

Applicants must provide a certificate of good conduct or police record from their local police department.

7. ADDITIONAL DOCUMENTS

If you are applying for the multiple entry visa, please provide the following documents in addition to the requirements above:

  • Current Certificate of Good Conduct/ background check issued by INTERPOL.
  • Legalized documentation of the constitution of the company that you represent in the United States.
  • Certified photocopy of the Tax Identification Number (NIT) or original electronic certification if the company was established in Bolivia.
