HE-11 Visa for Cuba Born Applicants

Requirements:


The following requirements apply to applicant who is Cuban born and left Cuba before 1971:

  1. A clear photocopy of the information page of your U.S. passport
  2. Completed Planilla para tramites consulares
  3. Completed Visa Processing Order Form
  4. Two passport photos
  5. Any document that shows that you left Cuba before 1970, such as school card, vaccination card, etc.
  • Embassy Fee $200.00 per applicant
  • Service Fee $99.00 per applicant
  • Return FedEx $32.00 per address

Processing time for HE-11 Visa usually takes 4 to 6 weeks for the approval.

Walk-in customers are welcome to drop off completed application 9:00 am to 4:00 pm, Monday to Friday. No appointment needed. Please note neither a public computer nor a printer for walk ins to use. Please have your application forms completely filled out and all required documents printed out before you come in. There may be a printing fee if excessive printing is needed. Note to walk-in customers.
