You must submit your actual passport. The visa will be stamped on a page in your passport. Your passport must have at least six months of validity left beyond your intended stay in Japan, and must have at least two completely blank visa pages. Pages marked "Endorsement" or "Amendment" do not count.

Photocopy of U.S. permanent resident card (front and back), or U.S. visa with Form I-94 is required. Applicant who is on U.S. F-1 student visa should also submit copy of Form I-20.

6. EMPLOYMENT LETTER
Employment letter should state your employment status with the company, purpose of visit in Japan, length of stay, and financial responsibility for all expenses. In addition, the letter should also state the name, address and phone number of the hotel or the person you are staying with in Japan.
7. BUSINESS LETTER (For Business Visit Only)
Business letter should state your position with the company, purpose of visit in Japan, length of stay, and financial responsibility for all expenses. In addition, the letter should also state the name, address and phone number of the comapny or person you are visiting in Japan.
